In NW Surrey we work together to plan and deliver services. Associate Director – Integrated Discharge
Discharging patients from hospital is a high priority for the NW Surrey Integrated Care Partnership and has been designated a transformation priority. In NW Surrey our ambition is to create an enhanced multi–organisational integrated discharge function, which will transform the way patients experience discharge from hospital. The NW Surrey ICP Partnership Board has approved the creation of a local joint Integrated Discharge Function.
We are seeking a talented leader to establish and oversee the work of the Integrated Discharge function and lead the strategic development of integrated care in North West Surrey. This is a new role and you will have freedom to develop the structures, systems and teams required to deliver the programme aims and you will have the full support of the ICP partners to do this.
We are looking for a leader who is able to build trusted and effective relationships across a range of stakeholders and has the ability to engage people and inspire them to work towards shared goals. The successful candidate will have extensive knowledge of NHS and social care policy and systems through experience of working across organisations and they will have proven experience of successfully applying change management and improvement methodologies within complex environments.
